FREE - PG Movie Permission Slip Form - Editable Word doc or Easy printable PDF

 

Description 

A simple letter to parents explaining some positive reasons to watch movies in class and to allow them to give their child permission to watch a movie at school. The blank areas included are:

- A place to write the movie name and rating

- A place for the teacher's name/signature

- A place for the child's name

- Tick boxes for Yes/No permission

- A place for parent's name, signature and date

I have included the original docx file and an old word format (97-2003) if you need to edit anything, as well as a pdf version for easy printing. Please let me know if you need anything else added via the ask a question.

Superworm Movie Guide + Writing Activities - Answer Keys Inc.

Description 

This 6 page movie guide with handouts accompanies the animation film "Superworm (2021)". This guide may also work well as a sub plan for your substitute teacher.

PART 1 – Comprehension questions in chronological order. I have made two sets for differentiation. The multiple choice questions may also work well with ESL/ELL students. Answer keys included.

  • 15 questions requiring full sentence answers.
  • 15 multiple choice questions with 3 possible answers.

PART 2 – Writing activities and Word Search

  • The first section students to write sentences using the beginning letters from the title SUPERWORM. I have given an example for the first one. Followed by writing about times the students felt happy, sad or excited while watching the film.
  • The second section is a word search puzzle with 15 words to find, 5 of which need clues answered beforehand. Answer key included.

The short film is based on the bestselling book by Julia Donaldson and follows Superworm, a super strong and super long worm who likes to help others.

Based on the book by Janell Cannon, the movie is about a baby bat named Stellaluna. After getting separated from her mother, Stellaluna ends up in a bird nest with 3 baby birds and does her very best to try and fit in.

PART 2 – Writing (younger grades)

• Students first write sentences using each of the letters from the word “IVAN”, then write about places in the movie where they felt sad, happy and excited. Followed by circling the correct spelling of 5 animal words (Gorilla, Seal, Chicken, Rabbit, Elephant)

• Favorite character writing - Students to write a little bit about their favorite character in the movie. Includes describing them, writing about their personality and what the student liked and disliked about them. Also need to draw them.

PART 3 – Word fun and word search

• There is a word bank with 15 words related to the movie to use in this part. For the first section students to unscramble the words from the word bank. Next is drawing a line through the letters spelling one of the words. Last of all is answering questions and clues to find the words.

• A word search with 15 words to find using the same word bank

PART 4 – Storyboard and Synopsis (older grades)

• Storyboard - Students to draw a 9 scene storyboard of what they believe to be the most important parts in the movie. To include a brief description for each scene. Remind students they can look at the movie questions to remind them of the sequences.

• Synopsis - Using their storyboards as guides, students to write a synopsis of the movie.

Marking idea for the comprehension questions

When the students have finished the questions, they swap their work with their friends to mark. We go through together as a class, taking it in turns to ask and answer questions. Students can also check grammar and spelling if more advanced, and then rechecked by the teacher.

Based on the novel of the same name by Katherine Applegate, the movie is about a silverback gorilla named Ivan who lives in a circus at the mall. With the help of his animal friends, Ivan tries to piece together his past and figure out how they can all live a better life.
