FREE - PG Movie Permission Slip Form - Editable Word doc or Easy printable PDF

 

Description 

A simple letter to parents explaining some positive reasons to watch movies in class and to allow them to give their child permission to watch a movie at school. The blank areas included are:

- A place to write the movie name and rating

- A place for the teacher's name/signature

- A place for the child's name

- Tick boxes for Yes/No permission

- A place for parent's name, signature and date

I have included the original docx file and an old word format (97-2003) if you need to edit anything, as well as a pdf version for easy printing. Please let me know if you need anything else added via the ask a question.

Return to Oz Movie Guide + Activities - Answer Key Included

Description 

This 15 page movie guide with handouts is for the film "Return to Oz (1985)". This guide may also work well as a sub plan for your substitute teacher.

PART 1 – Comprehension questions in chronological order. I have made three sets for differentiation. The multiple choice questions may also work well with ESL/ELL students. Answer keys included.

PART 2 – Storyboard and synopsis & character writing

  • Students to draw a 9 scene storyboard of what they believe to be the most important parts in the movie. To include a brief description for each scene. Remind students they can look at the movie questions to help with the sequences.
  • Using their storyboards as guides, students to write a synopsis of the movie.
  • Students to choose one of the characters listed and write a little bit about them and include a drawing.

PART 3 – Map of Oz and describing the route

  • Students to draw a map of their perceived perception of Oz based on what they saw in the movie. There are 8 key landmarks that need to be included.
  • Students to draw the route Dorothy took from her arrival to Oz to the Nome King's Mountain. Students then go on to describe the journey based on how they think it might have been for Billina the chicken.

Based on L. Frank Baum's early 20th century Oz novels, mainly The Marvelous Land of Oz (1904) and Ozma of Oz (1907). The movie follows Dorothy, who after being away from Oz for a while finds herself being sent to a special doctor to help her forget. Saved by a mysterious girl, Dorothy is somehow transported back to Oz and a new adventure begins.

  • Continuing on from the previous assignment, students to write about the habitat and daily routine for the magical creature. To include a picture of its dwelling place.

Based on the books by Holly Black and Tony DiTerlizzi, the movie follows the adventures of twin brothers Jared and Simon Grace and sister Mallory. After moving into the run-down Spiderwick Estate with their mother they find themselves pulled into an alternate world full of faeries and other magical creatures.

  • Students to design the layout for the new owl sanctuary as mentioned in the movie by Mother Paula. They will need to include the entrance, walking areas, souvenir shop, owl living area and any other important features, and also a key. An area below the design is for any other information student wish to add about the design.
  • Students to design a poster for the opening of the new owl sanctuary. They will start with the poster layout which will include numbered placements:
  1. The name of the sanctuary.
  2. The date of the opening.
  3. At least one picture of an owl.
  4. Special events for the day.
  5. A short paragraph about the owls and the importance of protecting them.

- Once completed students to create the final poster either on Letter / A4 / A3 paper

Based on the novel by Carl Hiaasen, the movie tells the story of young teenager Roy Eberhardt, who stumbles across a population of endangered owls threatened by the construction of a new pancake house. Making friends with two other likeminded kids, the trio make plans to try and stop the construction and save the owls.

Based on the novel by Beverly Clearly, the short movie is about a young boy called Keith who meets a talking mouse named Ralph at a hotel. Ralph takes an immediate liking to Keith's toy motorcycle and likes to ride it making motor noises as he goes.

Marking idea for the comprehension questions

When the students have finished the questions, they swap their work with their friends to mark. We go through together as a class, taking it in turns to ask and answer questions. Students can also check grammar and spelling if more advanced, and then rechecked by the teacher.

Based on the novel of the same name by Katherine Applegate, the movie is about a silverback gorilla named Ivan who lives in a circus at the mall. With the help of his animal friends, Ivan tries to piece together his past and figure out how they can all live a better life.
